Program Terms and Agreement

This form serves as documentation that College of Liberal Arts and Sciences (CLAS) Early Start participants are aware of, and agree to abide by, the CLAS Early Start policies and procedures listed below. Failure to do so may result in immediate removal from the Early Start program or other disciplinary actions deemed appropriate by University Housing, the College of Liberal Arts and Sciences or The Office of Student Rights and Responsibilities.

  1. Students will abide by the ABOR Student Code of Conduct, University Housing, and College of Liberal Arts & Sciences policies and procedures.
  2. Violation of the following "Zero Tolerance" policies will result in immediate dismissal from the program: * Students will not participate in any activities involving the use or presence of alcohol or other drug substances. * Students will not possess any items considered to be weapons of any sort. * Students will respect person and property. Damage, theft and physical violence are not tolerated.
  3. ASU is tobacco-free. Students may not smoke cigarettes or other tobacco products in the residence halls, classrooms or during CLAS Early Start activities.
  4. Students will attend and be on time for all events on the CLAS Early Start Schedule, including, but not limited to, classes, presentations, meetings, study sessions, residence hall meetings (if participant is living on campus) and activities.
  5. University Housing, Student Rights and Responsibilities and the College of Liberal Arts and Sciences reserve the right to remove students from the residence halls and/or campus.
  6. If you are no longer able to participate in the program, you will need to cancel no later than July 24, 2017 by contacting Ashley White, Assistant Director of First Year Programs by email at Ashley.R.White@asu.edu.
CLAS Early Start Liability Waiver
In consideration of my participation in the ASU Early Start program, I the undersigned assume the risk of and hereby release and forever discharge the Arizona Board of Regents, Arizona State University and the College of Liberal Arts and Sciences for any injury which I may sustain as a result of my participation in the CLAS Early Start program, and which results from causes beyond the control of, and without fault or negligence of, the Board of Regents, Arizona State University or College of Liberal Arts and Sciences and all its employees. If I am under the age of 18 years, I certify that I have obtained the signature of my parent or guardian allowing me to participate in the activities and that my parent or guardian has full knowledge thereof. Specific activities may involve some risks and dangers. I am aware that unanticipated and unexpected events may occur that may result in injury to me. I hereby assume all risks of injury that may be sustained by me in connection with the activities. I understand that it is my responsibility to obtain all necessary permission to participate in all associated activities or to verify that such permission has been obtained on my behalf. I represent that I have chosen to participate in the above activities voluntarily.
Photo and Video Release
I give permission to release my name, photograph, video and related information about me or a photographed/videotaped event in which I am pictured to the news media. I further give permission for ASU Public Relations and the College of Liberal Arts and Sciences to use this information and/or photograph in university publications and publicity in support of the University and the CLAS Early Start program.
Health Emergency Care - Permission to Transport/Treat
In the event of a medical or psychological emergency, I understand that the CLAS Early Start or University Housing staff will obtain emergency services, (i.e., ambulance, emergency room care, EMPACT crisis team, doctor care) to respond to the well being of CLAS Early Start participants. The ASU Health and Wellness Center will be utilized during regular business hours in non-emergency situations.
